CCNC Efforts Cited by RWJF. An article on the website of the Robert Wood Johnson Foundation’s Reducing Disparities at the Practice Site initiative (RDPS) notes the critical role small primary care practices play in caring for low-income individuals with chronic conditions. The article highlights the importance of a long-commitment to working directly with small practices if Medicaid is to address the medical, behavioral, and social needs of its patient population.
