CCNC lauded as “state of the art” program. In recent testimony to a committee of the N.C. General Assembly, new Division of Medical Assistance Director Carol Steckel, referred to CCNC as a “state-of-the-art program” and a “model for states all across the country” on which North Carolina should build as it reforms the Medicaid program. NPCC wins national video contest. Congrats to Northern Piedmont Community Care on being named one of our five winners nationally in the Robert Wood Johnson Foundation’s Transitions to Better Care video contest. You can view the winning video here (scroll to the bottom of the page). The NPCC team was also featured in a short film aired during a Feb. 13 “Care About Your Care” event in Washington, D.C. Says the RWJF: “The results being achieved in reducing readmissions and improving care transitions are impressive – and we’re excited about telling this story.”

CCNC, NC featured in AHRQ materials. AHRQ’s CHIPRA Evaluation Highlight, a summary of the national evaluation of the CHIPRA Quality Demonstration Grant Program, is now available on the AHRQ Web site. The Highlight describes the early accomplishments, challenges, and lessons learned in North Carolina, Maine, Massachusetts, and Pennsylvania.

CCNC in the Journal. Don’t miss the latest issue of the North Carolina Medical Journal, which includes articles by – or about – some notable CCNC staffers, including Carolyn Sexton, Dr. Jane Foy and Dr. Marian Earls. The full text of the issue is available here (PDF).

 

Other News

New report identifies “frequent flyers” across hospitals and CCNC networks. CCNC is now providing NC hospitals with lists of “ED frequent flyers” (high utilizers of Emergency Department services). Lists include Medicaid recipients with more than 10 ED visits in the last year, regardless of network assignment. These reports on are available on the IC Reports Server at:
[Network Standard Reports] > [Patient-Level Utilization Reports] > ED High Utilizer Report by Hospital.
After selecting the hospital, the report will return a list of all Medicaid patients with more than 10 ED visits in the past year, where at least one of those visits was to the selected hospital (a higher visit threshold can be set if desired). We are pleased to provide these reports to CCNC networks and their hospital partners working to manage ED usage.

Updated Chronic Care Reports released. CCNC's Chronic Care Reports have undergone some improvements, including: a “Population” parameter has been added to select either Medicaid or Health Choice recipients; new fields can be pulled from the Master Priority Table, the date of birth now appears in standard date format; and priority indicator definitions open in a separate report. We hope the changes are helpful to our users.
