Props in the Windy City. CCNC pops up somewhat unexpectedly in a Chicago Tribune story about efforts to save Medicaid costs through care coordination.
NC Practices begin connecting to NCHIE. Through the North Carolina Program to Advance Technology for Health (NC PATH), four free clinics and four CCNC-participating practices have installed electronic medical record systems and connected to the North Carolina Health Information Exchange (NCHIE). Congratulations to the following CCNC practices: Carolina Pediatric Group (Carolina Collaborative Community Care); Legacy Pediatrics (Carolina Collaborative Community Care); Chatham Pediatrics (AccessCare); and Choice Pediatrix (Community Care of the Sandhills). Events
CCNC in NYC. CCNC Director of Evaluation, Carlos Jackson, Ph.D., will be speaking at a one-day seminar on June 14th in New York City hosted by actuarial experts Treo Solutions. Carlos will highlight CCNC’s innovative approach to using clinical risk groups (CRGs) for risk-adjusted reporting, identifying individuals for the priority patient list, and will demonstrate how our program saves money and achieves results. NCHIE Buzz tour. Speaking of NCHIE, leaders of that organization will be conducting a statewide “Buzz Tour” this month to explain how NCHIE will promote the access, exchange and analysis of health information. Tour stops include many cities and towns across the state. For more information on the tour, see the NCHIE website.
CCNC Web Cooperative moves forward. The nine CCNC networks participating in CCNC web cooperative launched demo websites this week; final sites won’t be far behind. Look for new and updated web offerings from: Carolina Community Health Partnership, Community Care of Southern Piedmont, Community Health Partners, Community Care of Western North Carolina Community Care of Greater Mecklenburg, Northern Piedmont Community Care, Community Care of the Lower Cape Fear, Community Care of Eastern Carolina and Partnership for Health Management. Links to the new sites will be provided as each goes live.
New CCNC maps online. There have been a few changes to the CCNC network territories. Updated maps are available on our website. Please be sure to use these updated maps in PowerPoint slides and other presentations.
New behavioral referral forms now available. Please see our website for new forms designed to facilitate the referral of patients to and from behavioral care. It is hoped that the new forms will help foster easier initial communications between primary care practices and behavioral health care providers.
