Diabetes Navigator. Don’t miss this great story in the Salisbury Post on diabetes navigator Becky Slone, working through the auspices of Community Care of Southern Piedmont and the Beacon Community. The online story link is mysteriously dead, but you can find a text version here.

Northern Piedmont Community Care has produced a terrific overview video that shows how their efforts impact community health. You can view the video on the CCNC site (scroll down to the bottom of the page).

Winston-Salem meal program aids health efforts. The Winston-Salem Journal details an innovative Medicare project through which local nonprofit Senior Services of Winston-Salem provides meals to local seniors facing health challenges. The collaborative effort includes Wake Forest Baptist Medical Center, Forsyth Medical Center and Northwest Community Care.

More practices achieve PCMH. The Whiteville News Reporter story notes a number of practices working with Community Care of the Lower Cape Fear have achieved NCQA recognition as Patient-Center Medical Homes. Congrats to Lower Cape Fear and all the practices involved!

Transitional Care.  A recent article in the Triad Business Journal references the transitional care efforts of Northwest Community Care.

Pregnancy Medical Home Care Pathways. The CCNC OB Champions have produced two care pathways in the past 3 months, Management of Hypertensive Disorders of Pregnancy and Progesterone/Cervical Ultrasound Screening for Preterm Birth Prevention. These pathways, aimed at the 1,600 providers in the Pregnancy Medical Home program, seek to standardize care to promote patient safety and improved birth outcomes.
