CHCS Super-Utilizer Summit. In February 2013, the Center for Health Care Strategies (CHCS), in partnership with the National Governors Association, hosted a Super-Utilizer Summit to explore how Medicaid could best advance models for a high-need group of patients. CHCS recently released a new report that presents the Summit's common themes and key recommendations for building better systems of care for high utilizers. Events
Improved Care Coordination Conference. The Northwest Triad Coalition for Improved Care Coordination Conference will be held Nov. 1 from 8 a.m.-4:30 p.m. in Clemmons. Dr. Dobson will be a keynote speaker for this event which encourages community stakeholders to collectively develop solutions and benchmarks to improve the coordination of care for patients and residents as they transition across the continuum of services provided by various healthcare and community-based services. Other News
Community Health Award. CCNC/AccessCare–Robeson County has been nominated for a Community Health Award by Southeastern Health, Community Health. They were nominated for the work of the embedded transitions team within Southeastern Health and the existing partnership between the inpatient team and community partners. The awards ceremony was held at Robeson Community College on Oct. 15.
ACCP Award. Congratulations to Dr. Kim Thrasher on receiving the Ambulatory Care PRN Member Achievement Award at the recent American College of Clinical Pharmacy Meeting. This honor is awarded in recognition of outstanding and sustained contributions to the advancement of ambulatory care clinical pharmacy practice. Well deserved, Dr. Thrasher!

PCMH Expert. Congratulations to Marianne Ferlazzo, QI Specialist for AccessCare, who recently passed the exam for NCQA’s Patient-Centered Medical Home Certified Content Expert. Certified NCQA Patient-Centered Medical Home Content Experts receive thorough training and demonstrate in-depth knowledge to assist organizations applying for NCQA PCMH Recognition.
PMP Credential. Congratulations to Monique Poole, Project Administrator/Trainer for AccessCare, who recently passed the Project Management Professional (PMP) exam. The PMP credential is the most important industry-recognized certification for project managers administered by the Project Management Institute.
