CCNC's Practice Transformation Network practice lauded by CMS
West Caldwell Health Council, one of the practices enrolled in CCNC’s Practice Transformation Network (PTN), was praised by the Centers for Medicare and Medicaid Services (CMS) for its “exciting interventions [in] patient access to care.”
West Caldwell Health Council, one of only 70 high-performing practices interviewed and highlighted by CMS nationally, used a mobile bus to provide primary care services for the local elementary and high school. The practice, located in rural western North Carolina with offices in Lenoir and Collettsville NC, also launched a community college clinic to improve access to primary care for students. “Interventions like these make it easy for those who often don’t receive regular care to get the preventative care and treatment they need,” an excerpt from TCPi’s blog post reads.
CCNC’s PTN works with practices such as West Caldwell Health Council through ongoing coaching support and progression through five designated phases of transformation to help them transition to high-value, high-quality care.
