CCNC's PTN Behavioral Health Learning Collaborative Recognizes Practice Success in Transformation
“Preparing for Value Based Care” was the theme of the guided learning collaborative on June 20, 2019, where approximately 40 behavioral health professionals met at CCNC’s Raleigh location. These behavioral health professionals discussed their work as participants in CCNC’s PTN and how to utilize key concepts and change drivers from Practice Transformation in preparation for state and federal level healthcare reform efforts, such as Medicaid changes in our state.
This monthly learning collaborative began in the fall of 2018 and this meeting was the culmination of a year commitment to provide a relevant forum geared toward behavioral health providers focusing on key transformation milestones as a collective group. The collaborative engaged various agencies and organizations and providers were highlighted each month to share their successes and challenges working through the rigorous steps and quality improvement processes of Practice Transformation. Additionally, each webinar featured senior faculty expertise on how to approach various aspects of healthcare transformation relevant to behavioral health providers.
The final meeting convened the participants from the behavioral health collaborative in person, along with the senior faculty and CCNC leadership. Faculty who presented included Jennie Byrne, MD, former deputy medical director for CCNC, who spoke on utilizing the Collaborative Care Model, an evidence-based way to coordinate care between primary care and behavioral health providers. Andrew Clendenin, MSW, vice president of clinical strategy and innovation, gave the keynote address and highlighted recent state level changes around Medicaid reform as well as the importance of joining a clinically integrated network like Community Care Physician Network (CCPN).
As CCNC’s 4-year Practice Transformation Network work concludes in September 2019, several participants were recognized and eager to celebrate their successes as recent graduates of CCNC’s Practice Transformation Network.
